Guidance of the first visit

  • To receive medical care, you need a Japanese health insurance card.
  • Our hospital is based on the reservation system by referral.
    It is necessary to have a referral letter from the doctor who is currently taking care of you.
  • Before coming to the hospital, an appointment must be made by phone.
    Please prepare the referral letter before contacting us.

  • If you need an interpreter, we will guide you to the company that will introduce you.

What you need on the day of your first visit

  • Japanese health insurance card
  • Referral letter (Medical Information Form)
  • Information of image examination (CD, DVD, X-ray film, etc.)
  • Medicine information

Procedures at the time of first visit

  • Outpatient reception hours  8:30-11:30 a.m.
    (If there is a time specification when you make a reservation by phone, please see a doctor at that time.)
  • Please ask the general information on the 1st floor. The guide will inform you the reception procedure.
  • After filling out the application form, please take the numbered ticket of the first visit reception and wait.
  • If your number is called, please present your application form, health insurance card, and a referral letter.
  • We will give you a set of questionnaires, so please fill in them. When you have completed the form, please present it to the first visit reception desk.
  • Once you have a medical record ready, we will give you a green file with a patient ID card and a call card.
  • If you have image inspection results, we will accept the outpatient guide on the 2nd floor and import images to electric medical record.

Outpatient examinations

  • After completing the procedure on the 1st floor, please come to the outpatient information on the 2nd floor. We will perform body measurement and guide you through the usage of the call card and the flow of consultation.
  • We will inform you that the order of examination and treatment is approaching and when to enter the examination room with the sound, vibration, and message of the call card.
  • If you get a message on the call card, please come to the waiting room.
  • Touch the call card to the arrival confirmation machine installed in the waiting room so that the doctor can confirm that the patient has arrived at the waiting room.
    Please note that you will not be able to call the examination room unless you can confirm your arrival.
  • When the next message appears, please enter the room.

Accounting

  • After all examinations, please submit your green file at the accounting desk on the 1st floor.
  • After the accounting calculation is finished, we will contact you with a call card. Please come to the accounting counter.
  • Please pay at the automatic payment machine or accounting counter.
    Note: The following credit and debit cards are accepted.
Accounting

About prescriptions

  • Our hospital provides out-of-hospital prescriptions.
  • Please give a prescription at the accounting counter for confirmation. You can receive the medicine at the out-of-hospital insurance pharmacy with the prescription.
  • If you are instructed to receive medicine at an in-hospital pharmacy, the voucher number will be printed on the receipt after the accounting is completed. Please bring the receipt to the hospital pharmacy counter and receive the medicine.

Inspection reservations

If you are instructed to perform PET-CT examination, CT examination, MRI, endoscopy, or ultrasound, please make a reservation for the examination date at the reception desk J-5 on the first floor and explain various precautions when inspecting. If your doctor gives you an examination instruction, please stop by the reception desk on the 1st floor J-5 before returning.

Access guide

  1. By train and bus
    1. From the North Exit of Futamatagawa Station on the Sotetsu Line (1st floor of Sotetsu Life), take the Sotetsu Bus "Driver's License Office (Cancer Center) Route", get off at "Light Center Mae" stop. (Travel time approximately 5 minutes) or take the Sotetsu Bus bound for "Asahi High School Entrance Route", and get off at "New Town No. 1" stop (about 5 minutes).
    2. A free patient-only shuttle bus is available. Board the bus at the North Exit of Futamatagawa Station on the Sotetsu Line (1st floor of Sotetsu Life).
  2. By car

    Get off exit Honmura of Hodogaya Bypass and go along Atsugi Kaido toward Atsugi. Turn right at intersection "Driver’s License Center Entrance " (driving time approximately 5 minutes from Honmura Exit).

  3. On foot

    Approximately 15 minutes from the North Exit of Futamatagawa Station on the Sotetsu Line.
